Central Restistry for Convicted Persons:
ms.gov.pl/krk/krk.php

Biuro Informacyjne Krajowego Rejestru Karnego
Address:
ul. Czerniakowska 100,
00-454 Warszawa
Polska [Poland]
phone: (022) 39-76-200
fax: (022) 39-76-205
(there are also regional branches)

you need to submit (in person or by mail) this application:

ms.gov.pl/krk/form_krk_osoba.pdf (PDF for Acrobat Reader)
ms.gov.pl/krk/form_krk_osoba.rtf (RFT for MS Word)

and pay 50 Polish zloty, but the problem is the money has to be paid by buying special stamps (like post stamps, but available only at their offices or at courts through the country), so I don't know ho wyou can do it.

Edit: I changed one link, because the RTF document was for companies, not private persons.

I can't help, I don't live in Warsaw and I don't go there often.
Maybe make another thread and specify in the title that you need a favour in Warsaw.

Just go to the Polish Embassy or consulate and request the Polish Police Certificate (or in polish "Zapytanie o karalność") there.

I know this is a couple months late, but I just happened to stumble on this website.

As far as I was told in Kielce, all you need is to fill out and sign a form requesting information pertaining your criminal record, along with the payment, a letter explaining your situation that you are abroad requesting them to send the letter and funds for postage.

Call the Warsaw criminal registry or perhaps a smaller cities informational point to see if they will in fact oblige to your request.
