Baptism Certificate - Can anyone read this? Help!

I didn't know that we have a baptism certificate that shows the town/district/province of where our family is from in Poland, but now that I have it, I can't read it! I'm attaching a pic. It shows the civil geographic information.

I would REALLY appreciate it if someone could tell me where my family is from! :)

Province (Województwo) -Rzeszów District (Powiat) - Kolbuszowa Town (Miasto) -Dzikowiec Still there :)
