Media Survey on Polish immigrants in Chicago area

I am a college student working on my masters degree on mass media at the University of Central Missouri. Right now I am doing a study on media and Polish immigrants in Greater Chicago area. This is an invitation to take part in a survey online or hard copy questionaire. It should only take 15 minutes to complete. The survey is in both Polish and English. Please let me know which version you would like to fill out. I need to have this study completed by March 31, 2008 so I need your help now. I'm afraid I'm not from Chicago and can't take your survey but I just wanted to say that I went to the University of Missouri-Columbia which is not far from Central Missouri. Now that I've moved away, It's always good to hear from people in the middle of MO! Good luck on your survey.
