Sole trader registration

Hello, I'd like to start some trading activities in Poland (import/export) and I guess the easiest way is to be registered as sole trader.

I live in Poland so I have my address here as well as bank account, NIP, etc. What should I do to be registered as sole trader? Where? Do I need any kind of deed? Notary?
