taxation documents - help with PIT 37 required.

Hi guys and girls,
I am writing here to check if somebody can help me with TAX document called PIT 37. As full time employee I am obliged to fill this form. Can somebody of you help me how to fill this form. I got all the required documents from my employer payslips, PIT 11. If there is any tutorial in english how to fill it by myself I would appreciate somebody to share it with me.

If there is office where they can do that for me it is also greatly appreciated, if name the place.
I know that the deadline is aproaching ...;(
