Interpreting a Polish wedding form to fill in....

Hi, we got married in poland and we must get our priest here to fill in this form. However, we haven't a clue what we should beside "Lp. prot." or "L. dz."

Form attached... does anyone know? edit: can't attach the damn form...!

Lp. stands for "Liczba porządkowa", order number in table rows: 1st, 2nd, 3rd...

L. dz. i'm guessing that's "Liczba dzieci", no. of dependent children.

Would be much easier having a context.
Could you scan that form and post it over?

Mystery solved. Thanks to those married.
There are two books in your parish.
One is called marriage protocol (Protokół małżeński).
The second is diary (dziennik).

Lp.prot. is order number in marriage protocol.
L.dz. is order number in parish diary.

Marriage protocol is a document you have to sign before the marriage. Kind of marriage agreement. that you understand what marriage is about, sin, pre-marriage sex... :)

This "Zawiadomienie"/ document is for a priest only to fill in and to stamp & sign it at the end - not you.

The priest from a Parish where you got married will know what to write in lines:
"Lp.prot. - "liczba porzadkowa protokolu"- states recorded details about your marriage in (Parish) protocol"
"L.dz." - "liczba dziennika" - states recorded details about your marriage in (Parish) diary/book".

From the document, my guess is:

The whole document is addressed from one priest (from the Parish where you've been married) to a priest from a Parish where you've been baptised - it's all about to keep church record in order and updated.

This document addresses your Parish (the one where your baptism is recorded), and informs them about:
-your marriage, also asks your Parish to record the marriage in their books, and to fill in under the "Odnotowano w Parafii - Recorded in Parish" their recorded details, and to return the document to the Parish- where you got married.

After exchanging the information, both Parishes are going to have your church records in order and updated.

I couldn't edit my post to add this:

If the Polish priest gave you this document to fill , he wanted you to put a church name and address (the church where your baptism details are) under the: "Do ksiedza proboszcza parafii rzymskokatolickiej w...." - To a priest/ head of Roman-Catholic Parish in.."
