The town name is Brzeg nad Odrą Ulica (street) Swierczewskiego has been renamed Lwowska. This last snippet I found for you on

Here is the website, its in Polish. I would suggest you try www.dogomania.pl and once you got then add the rest namely /forum/f85/kochany.../index210.html

There is someone on the site who is able to say exactly where the old street and its numbers are found against the new street name.
