Brzozow, Poland

My Father past away Easter Friday 2006. I just found out two weeks ago. The reason I'm writing this is I need information from somone about this town, Brzozow. I need too write to the church and get information about where my father lived, physical address anything of this nature.

Could someone please, If you can, lead me in the right direction.
