I can't see the headline but then it's:
Powyższe ¶wiadectwo nie... stwierdzam ponieważ w międzyczasie żadne zmiany nie zaszły.
I can't read one word in the first line on the right side of this document. All this means something like that:
This document don't...I state because nothing changed in the meantime.

There are two words I can’t identify. I was only able to come up with this very poor translation. I thought it would at least give you an idea what this part of document says.

So here it is:

Polish : ‘Powyzsze swiadectwo..( 2 words I can’t identify)..stwierdzam poniewaz w miedzyczasie zadne zmiany nie zaszly”

English : Because no changes have been made / have occurred in the meantime, I certified...( Two words I cannot identify) the above certificate.

It appears this doc is some kind of a certificate signed by Mayor of Szydlowiec ( sorry but I don’t have Polish keyboard) who approved/ certified status quo of the doc/certificate as no changes occurred.

I hope it makes sense.

Thanks,

It makes sense... it's a health certificate (for travel) from my great-grandmother and was originaly issued in 1934 and the stamp after this coment reads 1935... (she left Poland in 1935)
