Help translating passport application form

Hello to you all. My name is Tomás and I am proud to be a new Polish citizen. I am going through my passport process and I need to know if there is any way I could get the form in English? I have to fill it out to get my Polish passport, at the embassy they gave me the form in Polish, but i don't speak the language well enough... any ideas on where to get the application or a decent online translator?

thankss i'm in big hurry.. I am attaching the form... In English it would really help.

there is no english version.. shouldn't you speak the language if you are a citizen? hmm

do you think that all, lets say new American citizens, speak English? lol, no, lawyers do that for them... ;)

they should, shouldn't they? and i was thinking yes, anyway.. as PL has an official language while USA doesn't.

The above posters are right. You cannot get a copy of this form in English. Your best option (as it has to be filled in using Polish Characters also) is to hire a professional translator, or a lawyer. There are various services advertised in different countries for this purpose. There is obviously a fee as with everything. This seems to be on average around £70, $100, or $100 CD. There are obviously lawyers in Poland as well who can help you with this. Try yourpoland.pl for a few places to look into having this done. Also try poland-citizenship.pl. If not have a look around the archives in the forum, as previous similar posts have received help this way, and also list some good websites.
The other option is to use an online translator to help you translate into English, and then translate your answer back into Polish, including characters. If you are going to try to do it this way, at least cross reference your translated answers with different sites to check you have the right spelling. The best online translator would be google translate.
Make sure your application is filled in block capital letters, as it states on the form. Not doing so will just lead to a denial of your application.

I'd reccommend you take the more official route of a Lawyer or Translator rather than trying to do it yourself.

or post your answers here for others to check for you. the online translators are not all that good, even the google one..
