Need English Translation Of 1912 Birth or Baptismal Document

It's a bit too small... generally It is a birth/baptism certificate stating that a child called Franciszka (female) was born on 17th February 1895 in Karwina (which I think is this town http://en.wikipedia.org/wiki/Karvin%C3%A1) in house number 380. There is also a name of the Roman Catholic priest and (in the table) of parents, godparents and midwife byt I have problems reading them... the document was produced on 3rd January 1912.

I got It downloaded and then zoomed It in... the father is Kaisar Franciszek, górnik (miner) and godparents are Kaisar Jan (also miner) and his wife Anna. The mother's name is in "Matka imię i pochodzenie" kolumn, I still can't recognize It.

Correction : The baby girl (Franciszka), according to the certificate of birth and baptism, was born on February 13, 1985 and was baptized on February 17, 1985.

Also, if you look at the second column ( the one next to Imię Dziecięcia column) it asks whether the baby belongs to a married couple or not and whether the baby is a son or a daughter.

According to the doc - the baby belonged to a married couple and it was a daughter(as indicated correctly by Grzegorz_)

The column next to 'Ojciec...' column asks about religion - the father was 'kat' ( Catholic) and than it asks about mother's religion - she was also 'kat' ( Catholic).

Child's name : Franciszka, daughter of
Father's name: Kaisar Franciszek, górnik (the miner), Catholic
Mother's first name: Ewa,daughter of ....Pryj???(not sure) Paweł , Catholic

Godparents names: Kaisar Jan, górnik ( miner) and his wife Anna
The name of midwife: Ms/Mrs Zięczek?? from Karvina,

was born on 13 February 1895 in town Karvina, in the house number 380 and baptised on 17 February 1895 in the Roman-Catholic church in Karvina by a priest .....(cannot read the name).

Below signed clerk certifies this document by signing and stamping.

Catholic Parish Office in Karvina, dated 09 January 1912 and signed by the priest provost (ksiądz proboszcz).

Karwina is in so called Zaolzie ie Western part of the Crownland of Silesia (Herzogthum Schlesien) which was claimed after WWI both by Poles and Czechs. Karwina was a industrial city with Polish majority however now is in Czech Republic.

I couldn't make the document bigger to see some of the unclear details but you've managed pretty well.
Just one thing, are you sure it's the 3rd not the 9th January?, because at the beginning of the document you can clearly see handwritten "3" (in 232 which states how many pages the registry book has - unimportant detail) and this 3 it's different to the one at the end.
In Polish handwritting "9"-nine doesn't have a stright line-leg, it ends more like with a half circle "leg" similar as in the letter "C" when reversed or is similar to handwritten "3"

Sure, I'm not ;-) It needs to see an original because to distinguish three & nine. Maybe you're right. But thanks God, it's not the most important fact here I hope. Thanks.
