you should always try to put some context if you wish things translated - of course you should omit your personal data anyway - sporządzona in this case (most probably) refers to the contract itself like 'umowa poniższa została sporządzona..' = 'this\ the following contract has been created (ie written down) ..' (in Polish a contract is umowa)
