Polish Prayer on a mass card

I found the card from my great grandfathers wake. The prayer is only in polish. I am not having luck translating it on line.
Can anyone help?

zwloki zlozone na cmentarzu Im. Jezus
Modlitwa
Najslodsze serce jezusa zawsze obecne
w przenajswietszym sakramencia,
zawsze przejete goroca milosci dla biednych dusz
uwiezionych w czyscu, zmilujsie nad dusza
Twego odeszlego slugi. Nie badz srogim w
twojem sadzie, lecz zezwol a by kilka kroel
Twej cennej krwi spadlo na pozerajace plomienie .
I ty, o pelen milosierdzia Zbawicielu
zeszlij. Twych aniolow aby zaprowadzili
Twego odszlego sluge do miejsca
ocholdzenia swiatla i pokoju.
Amen

Niech dusze wszystich wierzacych, zmarivch za laska boza odpoczywaja w pokoju. Amen

Sweetest heart of Jesus, ever present in the Most Blessed Sacrament,
Ever permeated with fervent love for the poor souls consigned to purgatory, have mercy upon the soul of Thy departed servant. Be not severe in Thy judgement, but permit several drops of Thy precious blood to fall upon the engulfing flames.
O merciful Saviour, send Thy angels to lead Thy departed servant to a place of of refreshment, light and peace. Amen.
May the souls of all the believing departed through the grace of God rest in peace. Amen
