Post office, customs forms, need help!!!

I need to ship something from Poland to an APO (American military post office) in Germany. I know I dont need a customs form for Germany, but its going from there to .... Somewhere else??? lets say!!!
I'm pretty sure I need a customs form, but my polish is a bit patchy, so can anyone tell me what I need to ask for??
So what I mean is, what is th Polish name for a customs form?? is there different types?? I suppose one that I'd use to go to the USA would be the correct one.........

Deiseaj: deklaracja celna This should do the job - they should understand this. But they should give you the customs declaration sticker automatically - if they don't, point at the United States part of the address and tell them the above phrase.
