Seeking a formal translation for Coast Guard ...

Hello,
I am trying to find some free help with translation of a two page document regarding my sea time spent aboard ships while employed in Poland. It is actually a simple table, only one page contains the actual data.
I have been out of work for quite a while, thus a search for a free translation.
The document that needs to be translated will be supplied to the US Coast Guard, to be a part of my merchant marine credentials,
