Short CV translation (less than 250 words)

Normally I would get this professionally done but since I'm out of work I really can't afford it so I thought maybe you kind folks would be able to assist me. I need to get my CV translated to Polish as I haven't held a Polish passport in over 10 years and need to submit a proof of Polish nationality to the voivodship I was born in through my country of residence Polish embassy. If anyone can help me out I'd love to send them at least a small thank you of something from Amazon.com .uk or whatever in the 20$~ range.

One major [if he is male, he can't describe himself as 'księgowa' ] and some minor corrections could be added to this translation of which the expression 'w dziale' rather than 'na dziale' [informal language should be avoided in a CV] can be mentioned. Also, I wouldn't use capital letters in the phrase "w magazynie Obuwia Ortopedycznego" [if I did, I would start 'w Magazynie'] and would say '... obejmowały przetwarzanie zamówień klientów ...' .
