i am preparing the In-Law's taxes here in Canada and need a quick translation for these key items... unfortunately (or not?) the better half is at work today. Przychód? Zaliczka na podatek pobrana i odprowadzona przez płatnika? Kwota składki na ubezpieczenie zdrowotne o którek mowa w. art...... odliczenie od podatku? Thanks in advance!

dochód is income

I don't know English accounting terminology - but google translate says 'zaliczka na podatek' is 'advance tax' (perhaps advance tax payed/contributed so far (in the stated year)

the last item is pretty Polish tax system particular - I guess you can still translate it as 'medical/health insurance paid (in the stated year)' (it is paid with every month in a stable job or for every job contract)
