Urgent help needed to translate memorial plaque Szari Szeregi

I am preparing this memorial plaque to take with me to Poland, but I have lost contact with my people there and I need a definitive text to make the plaque within the next 6 hours so the plaque get made before I travel!

Do you know anybody who could check the below and maybe correct it? The important part is the last 5 lines. I have a Google translation, but do not know enough Polish to be sure it is accurate.

Czuwaj! Szarych Szeregów - if you address them directly then "Czuwaj! Szare Szeregi" sounds much better for a Pole.

The last 5 lines in Polish should be rather, 3rd line is slight different:

Mieszkali tam, gdzie Ty mieszkasz,
kochali to, co Ty kochasz,
te góry, tą ziemię,
dla których oddali życie

Nie zapomnij o nich!

Mieszkali tam, gdzie mieszkasz (or żyli tam, gdzie żyłeś. Żyć=to live, mieszkać=to inhabit, both sounds alright for me)
Kochali to, co kochasz
te góry, tą ziemię, (only if it is a continuation of the above sentence, that is: they loved these montains, this land, if not you have to change the case of nouns)
dla których oddali życie.

Szarych Szeregów


Here's something wrong.
Członkowie Szarych Szeregów: (The members of Sz. Sz.), Polegli w Szarych Szeregach: (/the following people/ died in Sz.Sz.) , Służyli w Szarych Szeregach: (/the following people/ served in Sz.Sz.... I don't know what exactly you meant here.

if that suppose to be a vocative case, then yes, it should be like @translator wrote.
