First of all would this be the normal greeting for someone on their name day?

If so.. could you translate into Polish for me please,

One possibility is: Wszyskiego najlepszego z okazji imienin. (literally: all the best on the occasion of your name's day)

Really depends for who these wishes are but one of the most regural are:

Najlepsze życzenia w dniu imienin, spełnienia marzeń, zdrowia, szczęścia i samych sukcesów.
