Does anybody have details on exactly what this is, who is eligible for it and how to get it?

It is card for Ukrainain/Bielrussian/Russian/Kazahstan citizens who have Polish roots.

Owners of this card are not going to need Polish visa, they are going to get education for free, health care for free, cheaper public transport, free access to museums in Poland.
