What do you mean ?

I don't read Polish, so navigating a Polish site is difficult for me. If you could provide information on what to "click" on to get the USC addresses for different locations, it would be extremely helpful!

If you click on "Organizacja" you will find "Urząd Stanu Cywilnego" and under it will be the name of the woman who's in charge of it - Grażyna Nocuń.
You click on her and you will find her room number and telephone. I assume the adress would be the same as the one on the front page, since no other is given.

Yes. Looks like the whole USC over there is one woman in Urząd Gminy. Not surprising, It's not a town, It's a village, they don't even have streets' names.

It works for me and they are indeed different and It's hard to say which is a current one, but in the phone number there's only one number different (except 48 which should be added If you call to Poland from abroad), so looks like they just decided to use one of their other numbers as the official one and e-mail seems to be similar too, just on the other server.

Thanks for the additional informaton. At least the address is the same, so there isn't any worries about the request getting lost in the mail (not likely I'll call since I can speak Polish either).
