Polish addresses

Could someone explain to me how addresses work in Poland? Maybe a few sample addresses would be helpful. I am used to USA style: Name, Street, City/state/zip. When I look at addresses in Poland, some seemed to be arranged differently. The part of Poland I am most interested in is Czestochowa (if the area makes a difference).

Example:
Name: Krystyna Bolo
Street: Ul. Kurza Łapa 9/13
City: 01-213 Jajawice

Poland doesn't have states. They have districts (Powiat). Sometimes it can be written in the address as well

Ok, thank you. The 9/13 is the house number, why 2 numbers? Also, the 01-213, is this what the US would consider a zip code? Or maybe some type of area designation within the city?

The 9/13 is when a person lives in building nr9 apartment 13. sometimes it's written "9 m. 13". The "m" stands for mieszkania, which means apartment or home.
01-213 is the area code of that town. A small towm would have one area code, but a large town such as Warsaw has many different codes depending on the district.

Spiritus, you have any street info on Czestochowa in a format that can be emailed. Road maps or something of that sort?
