Is Maria still Kubińska in America?

Anyone know what the naming procedure is for Polish females with adjectival surnames in the US, Canada and elsehwere? If she has Kowalska in her Polish passport does that automatically become Kowalski in her American identity papers or is it up to her to decide?
If she is married, can her husband be a Kowalski whilst she remains a Kowalska?
Is she legally free to retain her original -a ending or must she undergo some bureaucratic procedures to do so?
