Sworn in translator needed [English into Polish]

I am looking for telephone numbers of sworn-in official translators, English into Polish. Does someone know a website where these are listed? And does someone know how much it would cost per word, more or less? I have 1 page (proof of medical insurance) - about 300 words, I guess.

80 PLN per page is roughly what I pay providing it does not exceed a certain amount of words.

(official data base of Ministerstwo Sprawiedliwości, which is the entity responsible for accreditation of sworn translators in Poland)
you need to choose one (or more) of the following filters:
- "język" - a language (English = angielski)
- "województwo" - a voivodship (Warsaw is in "mazowieckie" voivodship),
- Miasto (City)
- Nazwisko (translator's surname)

ukpolska, your contact was really excellent. She translated and certified my document for a very good price, and very effectively. I just sent it to Lublin and she sent it back. I'm very satisfied and will definitely use her services again when needed. Thanks again!!
