Telling the time and date

How do I tell the time in Polish?

For instance, in my akt urodzenia it is written> born at 14:22 on 25th of June of 1984 in this subdistrict?

How would that sentence be, not just repeating the numbers but writting the hour, the minutes, the day and the year?

I really appreciate your help

Urodzona (or Urodzony for male) 25 czerwca 1984 roku o godzinie 14:22 w tym subdistrict

Subdistrict is translated like in English?

example
Urodzona dnia dwadzieścia pięć czerwca??

Urodzona dnia dwudziestego piątego czerwca tysiąc dziewięćset osiemdziesiątego czwartego roku o godzinie czternastej dwadzieścia dwa.

Subdistrict is not translated like in English but I don't know proper translation.

I've had that problem here before. "Oh you don't need the numbers written out in full." they say. But I'm learning to SPEAK Polish!

Do you think 'w tym okolicy' would fit for subdistrict?

Firstly, w tej okolicy would be correct, although I don't think the term okolica is the best translation for subdistrict.

Dzielnica would be the correct translation for 'district', whereas the term osiedle could be used for subdistrict

okolica would translate best as surrounding areas/surroundings

If district is dzielnica so subdistrict must be part of the district, so maybe sth like dzielnica wewnętrzna
